General Waiver of Liability

Live Theatre Workshop Education Wellness and Safety Policy: 

Live Theatre Workshop is committed to maintaining a safe and healthy environment while supporting the educational and artistic process in the most practical and effective way possible. 

If you are ill, or you suspect you have been exposed to any contagious illnesses, we ask that you please stay home and return to class/camp only after you have been symptom-free for 24 hours without the use of medication.

We ask that you please contact Live Theatre Workshop by phone or email if you are going to be absent.

This policy is subject to change as everyone’s health and safety remain a priority.

Photo / Video Permission:

I  hereby grant / do not grant permission for any and all photographs and videos taken of myself at any Live Theatre Workshop class, camp, rehearsal, performance, or event to be used by Live Theatre Workshop or its representatives for marketing purposes. I understand that no personal information will be associated with any photograph or video without my consent and that no compensation is offered.

Live Theatre Workshop Refund & Cancellation Policy:

More than two weeks before start: Full refund minus $50 deposit.

Less than two weeks before start: No refund, but tuition may be credited toward another program within one year (subject to availability).

On/After start date: No refund. Credit may be issued for the same session. (subject to availability).

Illness-related cancellations: No refund. Credit valid for one year (subject to availability).

If Live Theatre Workshop cancels before start: Full refund.

If Live Theatre Workshop cancels during term: Refund for unused class days, calculated proportionally.

I confirm that all information provided on this form is accurate and complete to the best of my knowledge. I understand that Live Theatre Workshop reserves the right to restrict or cancel admission at its sole discretion.
